Abstract:

Back ground: muscle spasticity is a common disability syndrome after a stroke. It can manifest itself in different ways, including the loss of free movement of a limb, joint contracture and can severely affects the normal walking ability and activities of daily life. Reducing the spasticity is often an important problem in the management of patients after stroke. Many studies reported different findings about the effectiveness of acupuncture to control spasticity but its efficacy had not been proven yet with a comprehensive study.
Material and methods: In this clinical trial we randomly divided 60 patients with post stroke spasticity into two treatment groups: acupuncture (treatment group) and physiotherapy (control group). All of the patients received 5 times treatment per week for 4 weeks. the outcome measures used were modified Ashworth Scale (MAS), Clinical Spasm Index (CSI), Barthel Index (BI), gait and foot print analysis, and Patient-reported Outcomes (PROs). This study was conducted at the Dongzhimen Hospital of Beijing University of Chinese Medicine.
Result: Both therapeutic method significantly decreased the CSI of participants at the end of treatment(P <0.005 ),however acupuncture was significantly more effective to decrees CSI after 4 weeks of treatment(P<0.05).
The average of Ashworth Scale for lower limb significantly decreased from 2.73±0.82 before treatment to 1.86±0.81 after acupuncture treatment (ankle joint), and from 2.33±0.66 to 1.6±0.49 (knee joint) for treatment group. There were significant differences noted between the treatment group and control group after intervention and acupuncture was significantly more effective (P<0.05).
Both groups showed improvement in BI. However, the improvement in acupuncture group was better than control group(P<0.05). Our finding in foot print analysis showed: both treatments significantly decreased the difference of step length between affected and unaffected side but acupuncture was significantly more effective (P<0.05).acupuncture showed better effects of improvement of foot length and correction of foot angle(P<0.05) .therapeutic response about step length and foot angle just after 1 week treatment also were significantly better in acupuncture group (P<0.05) .
Each of two treatments decreased the stride width during walking but the difference between two groups was not significant. Acupuncture and physiotherapy increased the gait speed and there was no significant difference between them. 50% of patients were Very satisfied about the outcome of acupuncture in the end of treatment course and it was 6.7% for physiotherapy.
Conclusion: Acupuncture is significantly effective in reducing spasticity in post stroke patients, and could improve walking ability and gait parameters. It could thus be recommended as a safe and effective therapeutic method for post stroke patients. Besides, foot print analysis is a clinical and economical measurement method to evaluate of spasticity.

Were specific questions or concerns raised? : It was very intresting for them that in iran we have traditional medicine school , also development of complementry medicine was surprising for them. many question about the efficacy of acupuncture for post strok and situation of traditional medicine in iran.

Your experiences about the travel processes(Providing ticket, accommodation,...) : very difficult ti get visa. very very very expensive. i spned 4000 USD just for visa, ticket and accommodation. sydeny is one of most expensive city in the world. the conference provided good facilites for participants such as halal foods and transfer. that was very amazing for me that alternative medicine and acupuncture are very papoular in australia but very expensive.
it was a good experince for me to take part this conference and meetings because i met key people and experts from all the world to learn more specially in the field of education of comlementry medicine, i was elected as e excuttive memeber of world fedration of acupuncture as a representative of TUMS, that was a great achivment .
Please give a briefing of your own observations and outcomes of the congress: : it was a great chance and experince for me to take part this event, i was elected as a executive memeber of world fedration of acupuncture as a representative of TUMS, that was a big achivment for me and good chance to do some joint research. there were many participants from many countries ( about 50) , it means develepment of alternative medicine around the world . there were also many great speeches in this conference that gave me some idea to do some research. also i was elected as memeber of scintific committe of world fedration of acupuncture. i met many students there that they were intrested to know about iranian traditional medicine
