Abstract:

OBJECTIVES: Idiopathic achalasia (IA) is a chronic motor disorder of esophagus. Botulinum toxin (BT) injection reduces lower esophageal sphincter (LES) pressure and alleviates symptoms in IA. Recently, Ethanolamine oleate (EO) has been introduced for treatment of achalasia.
AIM: To compare the long-term efficacy of intrasphincteric BT and EO injections in treatment of IA patients.
METHODS: 220 IA patients were evaluated prospectively.21 patients who were unwilling to or poor candidate for pneumatic balloon dilation (PBD) and cardiomyotomy (CM) enrolled. They were randomly assigned into two groups. Eleven patients were treated by two injections of BT at four weeks interval. In another group, EO was injected 3 times with two weeks intervals. All patients were followed by achalasia symptom score (ASS) and timed barium esophagram (TBE). We defined good response as decrease in ASS (≤ 4), and reduction of height and volume of barium in TBE80% of baseline at 1.5 months after last injection. Relapse was defined as increase in ASS (4) after initial good response. 
RESULTS: Mean age of patients was 63.14± 13.2 years (Min: 26, Max: 81). All patients in EO group and ten patients in BT group revealed good initial response Five patients in EO and four in BT groups, who had relapsed, were treated by re-injection. The mean duration of follow up was 27.38 months. Finally, in BT group, six patients had good response (ASS≤ 4) and five had poor response, but in EO group good responses were nine and only one patient had a poor response.(P value: 0.049)
CONCLUSION: This study revealed that both BT and EO are effective in the treatment of IA, but in long term follow up more patients in EO group remained in remission. Moreover, the cost of BT is approximately 20 times more than EO. We suggest the use of EO in selected patients with IA.

Please inform us if there are any follow up actions we need to talk with the members of the congress : IIn conclusion, when we spoke about achalasia treatment and the limitation and risk of different treatments such as surgery and balloon dilation, participates had decided to do more research about Achalasia treatments. Because until now, there aren’t any definitely treatment, and because our center in Iran have more than 1000 achalasia patients, some countries such as Netherland are encouraged to have cooperation with us.
Your experiences about the travel processes(Providing ticket, accommodation,...) : Shanghai is historical place and its interesting for visiting.The accomodation was good ,but their food was awful for us.
Please give a briefing of your own observations and outcomes of the congress: : We participated in more than 44 oral presentations and saw more than 2000 posters. Some of them were about Achalasia, but none of them had the large number of patients similar to our center in Iran. Some oral presentations explained about new methods for achalasia treatments such as laparoscopy and Heller myotomy and injections of Ethanol amine olerate and botulinium toxin injections. 9 Working Party Reports, a distinctive feature of each World Congress, were chosen out of the 28 high-quality proposals submitted and their findings were presented over the 3 Main Program days. Several lunch and evening satellite symposia were held throughout the Congress, organized by the biomedical industry and open to all Congress participants.
