Abstract(Please copy/paste the abstract send to the congress) : Delayed diagnosis: related factors and outcomes in patients with ankylosing spondylitis

Background: Ankylosing spondylitis (AS) is one of the rheumatic diseases with the longest diagnostic delay. Improving knowledge about diagnostic delay status and related factors and outcomes may help clinicians diagnose AS in earlier stages.
Objectives: To assess the diagnostic delay, related factors and outcomes in an Iranian population with AS.
Methods: A total of 163 patients with AS diagnosed by modified New York 1984 were participated consecutively in a cross sectional survey. A pre-designed form was applied for gathering data. The measured outcomes were included: Bath AS functional index (BASFI), AS quality of life (ASQoL), Bath AS metrology index (BASMI), Bath AS disease activity index (BASDAI), chest expansion and sacroiliitis grading by pelvic radiography.
Results: The average (mean±SD) diagnostic delay was 7.88 ± 7.17 years. Diagnosis delay was longer in patients with enthesitis versus without enthesitis (P=0.007) and in B*27 negative versus B*27 positive ones (P=0.01). Educational level was inversely correlated with diagnosis delay (P=0.002, r= -0.24). Diagnostic delay was also correlated with following outcomes: BASMI (P<0.001, r=0.41), BASFI (P=0.003, r=0.23), ASQoL (P=0.008, r=0.21), BASDAI (P=0.03, r=0.18), chest expansion (P<0.001, r= - 0.38) and sacroiliitis grading (P=0.04, r=0.16).
Conclusions: Low education, negative HLA-B*27 and enthesitis are the factors affect the diagnosis delay in Iranian patients with AS. The grater delay in diagnosis of AS, the more likely that poorer QoL, worse function, higher disease activity and more severe limitation in spinal mobility would become apparent. National referral strategies for patients with enthesitis (particularly low educational level) should be provided to use by related physicians.

The Congress Reporting Form
How many volunteers were present at the Congress? : About 300 persons including speakers of main sessions, oral presenters of articles, poster presenters and representatives of drug companies were present at the congress.
Delegates from which countries presented in the congress? : The speakers of main sessions were from Turkey, Spain, Italy, France, Algeria, Greece, Russia, Malta, Morocco, Egypt, Tunisia, Portugal, Germany, and Croatia. The delegates from Some other countries also participated in the congress. I was the only delegate from Iran who participated in the congress with article.
Were the delegates of any other organizations present in the congress? : Yes
If yes, please write the names of the organizations in the box : Pfizer, Roche, ucb.
What were the responses to your talking points? Were specific questions or concerns raised? : There was no special question about my poster. But one of the rheumatologists from Turkey asked me about my past published manuscripts.
If you met staff members, please list their full names & positions. : Some of the members which I visited were as follows:
1- professor Hasan Yazici, Division of Rheumatology, Department of Medicine, Cerrahpasa Medical Faculty, University of Istanbul, Turkey. He had some lectures and also he had the session of “meet the professor” about Behcet disease which I had a small challenge with him about skin aphthous as a specific symptom of Behcet disease. He was also the honorary president of congress.
2- professor Nurullah Akkoc, Dokuz Eylul University School of Medicine, Izmir, Turkey. He was the president of congress
Please inform us if there are any follow up actions we need to talk with the members of the congress : It will be interesting if we could invite some members for future Rheumatology Congresses in Iran such as professor Hasan Yazici (Behcet man from Turkey) or Loreto Carmona (from Spain, editor in chief of Rheumatology International) or Maurizio Cutolo (from Italy who talked about EULAR ongoing initiatives).
Your experiences about the travel processes(Providing ticket, accommodation,...) : I had some problems and worries for paying registration and accommodation. Because of the economic sanction of our country, it was impossible for me to pay by credit card. I mailed to the congress organization and explained the problem. They let me to pay in cash on site of the congress with early registration fee (350 Euro). Also, for accommodation, I selected a Hotel near to the congress site and paid the fees through an Iranian tourism organization.
Please give a briefing of your own observations and outcomes of the congress: : That was my first experience from an international congress of Rheumatology. I saw the Editor in chief of “Rheumatology International” (one of the Rheumatology Journals with high impact factor) who had the pre-conference course about the “scientific paper”. I learned some points from her and also, some other European editors. I will apply these points in writing my future manuscripts. Also, I visited the professor Hasan Yazici (the Behcet man from Turkey and had a small challenge with him). Participation in international congresses will help faculties to be known better. This help them for publishing their scientific manuscripts in International high impact factor journals.
