Scope: It is estimated that pumps account for 7% of the total maintenance cost of a refinery, and pump failures which can cause accidents (fire, explosion), are responsible for 0.2 % of lost production. In order to prevent those accidents that result in unit shutdown, investigating pump failures and identifying the critical parts of the pump is a vital. The aim of the present study was to investigate failures of the main feed pump in the ISOMAX unit of an oil refinery by FMEA (Failure Modes & Effects Analysis) technique.

Method: After becoming familiar with the process of ISOMAX unit, the components of the main feed pump were listed from the technical drawings and the role of each component was identified through discussions with the members of maintenance group. The failure modes of each component were determined by studying the past 5 year's records and by obtaining the views of experienced responsible individuals. The failure rates, severity of all failures and their detection probability were determined as accurate as possible. From all published tables on the ranking of failure rates, severity and detection probability of failures, 3 new tables were designed. The FMEA work sheet was completed and the value of risk priority number (RPN) for all the identified failures were calculated.

Results: Totally 45 modes of failures were identified for 27 items of pump, the calculated RPN of which were ranged from 16 to 192. Regarding the refinery's policy which was insisted on continuation of production without any shutdown, those failures whose consequences were pump trip, were considered as critical failures. They were belonged to 4 items with RPN values of 90, 100, 100, and 180. For preventing pump trip and resulting accidents, periodic detection of vibration was the main proposition. This can result in servicing the pump and replacing the item in time.

What were the responses to your talking points? Were specific questions or concerns raised? : One question was about ISOMAX unit and one about Maintenance in Hydrocarbon Industry. Usually maintenance is planned according to past work of equipment with views from operator & maintenance group. But we were offering a Risk assessment technique which usually is a method in Safety engineering, to be performed and from the results to find the critical components and plan maintenance work as to prevent the failure of these critical components. This is very new in the world and that is why the question about it had a long discussions which took 10 minutes time and the person in charge of session interfered in discussion and asked to be discussed latter and after the session.

Please inform us if there are any follow up actions we need to talk with the members of the congress : One point I noticed and was very interesting to me and it was OIC ( Organization of Islamic Cooperation) and part of that was called Statistical Economic and Research and Training center for Islamic Countries (SESRIC) which is located in Ankara. They were representing themselves in conference and I think you can go to their website( www.sesric.org) and try to contact them by offering some research projects on Health and public health in all Islamic Countries. Apparently they have some funds and I myself can concentrate on Industrial Accidents and the total loss in different countries and also to work on practical methods for recording accident's statistics. We also can offer training workshops in at least Persian speaking countries suh as Afghanistan, Tajikistan and so. This is the only point you can follow up and produce a fruitful research projects and some training activities. Our students also can participate in vocational training programs which SESRIC has in different countries. For these vocational training we can offer experts for educating the participants.

Please give a briefing of your own observations and outcomes of the congress: : Organizing committee could do much better. The exhibition on PPE was very update. Companies from Japan, Taiwan,China, Uk, Germany, Italy and many other countries were actively in exhibition. In fact my interest was gone on those companies which have presented their products. One of these was working on testing of personal protective equipment and using the European standards. It was originally a Turkish company and I was absorbed on the list of their measuring instruments. They were testing for example helmets for welders, electricians and construction workers. They also was working on determination of ignition temperature of materials and fabrics. This is the field I am interesting and would try if I can to work with them in the future. Also a Japanese company was presented anti vibration gloves for those working with machines and industrial tools. I got a sample from them and would show it to Iranian producing companies.
