Successful performance of self-assessment is considered the first step of accreditation process which is very important. As a result, taking advantages of self-assessment forms leads to a systematic performance of self-assessment. The purpose of self-assessment forms is to collect the required data about each accreditation indicator. The required information includes three simple questions which are about the availability of documents, implementing the indicator and reviewing it. If a unit declares that it is observing an indicator, it should provide the supporting documents and if the unit declares that it is not observing an indicator, it should propose an improvement plan.
